Abstract:
A new concept in laparoscopic trocar systems is presented that could be used with most presently available laparoscopic trocars. The Ethicon XCEL 5 mm trocar is used as the basis for this presentation; however, any other size or system of trocars similarly configured would be compatible with the ZTSS being presented.During laparoscopic surgery trocars often slip out of their original placement through the layers of the abdominal wall surrounding the abdomen. This causes the access to the peritoneal cavity to be lost, often at a critical moment during surgery. This necessitates various maneuvers to replace the trocar which waste time and also predispose the patient to complications such as subcutaneous emphysema and bleeding. The ZTSS would prevent this from happening.

Abstract:
A device for anchoring a trocar is disclosed. The device may include a spacer and a device having a main body and a device sleeve. A device tip may be hingedly connected to a distal end of the device sleeve. The spacer and the device may be configured to be assembled together with a trocar and an obturator. The spacer may provide distance between the device tip and an open distal end of the trocar sleeve when the obturator, trocar, spacer, and device are assembled together. Once the assembled parts are inserted into a body cavity the obturator, trocar, and spacer may be removed. Inside the body cavity, the device sleeve may receive the trocar sleeve. As the trocar sleeve progresses through the device sleeve, the open distal end of the trocar sleeve may push the device tip from a first position to a second position, anchoring the device.

Abstract:
A collection bag for better isolating and removing a large mass of tissue from a female patient's abdominal cavity during a laparoscopic surgery includes: (a) a flexible and resilient ring that has a configuration adapted to: (i) enable it to be distorted so that it can be vaginally inserted into the patient's abdominal cavity and then quickly return to essentially the same shape that it took prior to being deformed, (ii) size the ring's open area so that the large tissue mass can pass though it, and (iii) be further distorted so that it can be removed from the abdominal cavity through a laparoscopic-surgery-appropriate incision in the wall of the abdominal cavity, and (b) a sleeve having a flexible wall, an open end, and a defined interior volume, and wherein the sleeve's open end is affixed to the ring, and its interior volume is sized to allow the excised tissue to reside and be isolated within the bag's sleeve.

Abstract:
An improved system for isolating and removing a large mass of tissue from a female patient's abdominal cavity during a laparoscopic surgery, includes: (1) a wound retractor platform, (2) a vaginally-insertable, tissue collection bag having a flexible and resilient ring at its open end that is distortable for vaginal insertion but quickly resumes its open and undistorted shaped within the patient's abdominal cavity upon insertion, and whose open end receives the large mass of tissue and is then connected to the wound retractor platform so as to form a sealed bag, (3) a tissue cutting instrument which cuts the large mass of tissue into smaller pieces and then aspirates them along with some of the surrounding fluid, and (4) a fluid recirculation system which filters and recirculates the aspirated fluid back through the bag.
